As a Care Manager, Telephonic Nurse, you will be a vital part of our caring community, ensuring optimal continuity of care for patients transitioning into and out of our services. You will serve as the primary clinical representative for CenterWell Home Health, acting as an ambassador for patients who have been discharged or those who may need post-acute services. Your role is to bridge the gap between hospital discharge and home recovery, ensuring patients are matched with the right care setting at the right time.

About the Role

In this work-from-home telephonic position, you will report directly to the Manager, Care Management. Your day-to-day involves navigating complex healthcare options, Medicare coverage, and billing issues to support patients in need of additional services. You will leverage your expertise in home health, hospice, and palliative care to provide education and clinical level of care determinations based on medical records and patient discussions.

You will communicate with empathy over the phone, acting as a customer advocate throughout the referral process to maximize admission conversion rates. Whether helping a patient locate a specific service site, resolving complaints, or facilitating follow-ups, your goal is to ensure timely responses and high patient satisfaction. You will also assist in the admissions process and participate in special projects to further our mission of whole-person wellness.
