About the Role

This Case Manager position is designed to support patients by coordinating care and connecting them with vital community resources. You will serve as a vital link between patients and the broader healthcare system, advocating for their needs and helping them navigate complex medical and social services. The role focuses on improving health outcomes by addressing critical issues such as housing stability, transportation access, food security, and behavioral health support.

In this onsite role, you will partner closely with medical, behavioral health, and social services teams to develop and implement individualized care plans. Your day-to-day involves conducting patient assessments to identify specific needs, facilitating referrals, and maintaining accurate documentation in our electronic health record (EHR) system. You will play a key part in patient engagement and education, ensuring continuity of care and helping individuals achieve their health goals.
