This position serves as a dedicated advocate and case manager for clients participating in a life management program. The role is designed to support individuals through complex, long-term case management, ensuring they can function effectively within their environment. By understanding the unique history of each client, the case manager develops tailored program plans and goals, evaluating progress through ongoing mentoring and counseling sessions.
The case manager acts as a vital bridge between clients and the services they need, securing resources to help them cope with their surroundings while ensuring strict compliance with all funding requirements. This role requires a balance of empathy and organizational skill to manage a caseload efficiently, maintaining clear records of client progress, feelings, and personal growth.
