Mentor Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. For over 50 years, our mission has been to help people live full, independent lives by providing quality services and individualized supports that foster growth, regardless of the physical, intellectual, or behavioral challenges they face. Today, our 40,000 team members continue to innovate and enhance care for the 50,000 individuals we serve across the U.S.
You will join a mission-driven team where every person has the right to live well and everyone deserves a fulfilling career. In this role, you will create meaningful relationships that motivate us all every day. You will provide direct support within a residential setting or periodic program, typically assisting 4 to 6 individuals (or up to 10 in periodic programs) with their daily lives.
Your day will involve partnering with individuals to achieve their personal goals through socialization, health maintenance, and skill development. You will assist with activities of daily living such as personal care, housekeeping, recreational activities, transportation, and community integration. Whether accompanying individuals to medical appointments, administering medications, or preparing meals according to specific dietary plans, you will ensure their safety and well-being while respecting their rights and choices. You will also maintain accurate records, manage individual assets within budget, and foster healthy relationships with families, guardians, and case managers.
Candidates must be at least 18 years of age and possess a valid driver's license in good standing. All applicants are required to undergo a criminal background check, including checks against the Central Registry of Offenders Against Individuals with Developmental Disabilities and the Child Abuse Registry Information (CARI).
