As a Care Navigator, you will become a pivotal part of CareConnectMD's interdisciplinary team to collaboratively manage complex patient panels. The team is led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, and Care Navigators.
Because many of our patients are frail and elderly, we deliver care primarily in long-term care facilities and their homes. Many patients and families lack constant and reliable communication regarding their health status and care plans. You will serve as the point of contact for the "CareConnect Concierge" program, using your critical thinking and communication skills to build lasting relationships and earning trust through compassion and empathy. You will help patients and families understand the value of the CareConnect Program, which is offered at no incremental financial cost.
Your day will involve a mix of 40% home and long-term care facility visits and 60% in-office phone-based calls. You will conduct structured surveys to collect clinical and psychosocial data, partner with the clinical team to drive improvements in patient outcomes, and contribute valuable insights to weekly interdisciplinary team discussions.
