We are seeking a Care Manager - Memory Care (RN or LPN) to provide leadership and oversight for resident care coordination, documentation quality, and care plan development. This role focuses primarily on our Assisted Living Level 1 and Memory Care Neighborhoods, integrating clinical supervision with care management to ensure compliance and high-quality outcomes.
As a Nurse Supervisor on duty, you will serve as a vital link between clinical teams and families. Your day involves conducting care conferences, managing order entry, performing clinical triage, and coordinating appointments. Beyond administrative duties, you will provide direct supervision and support to nursing staff while assuming direct nursing care responsibilities, including medication administration and wound care, when needed.
