The Care Management RN within the Family Medicine Center Clinics plays a pivotal role in coordinating the care and services of selected patient populations while ensuring the efficient utilization of healthcare resources. This position is accountable for assessing, planning, implementing, coordinating, monitoring, and evaluating the options and services required to meet patient healthcare needs, all while promoting quality outcomes and patient satisfaction.
Depending on your specific education, experience, and license requirements, this position may be filled as a Level 1, 2, or 3. As a Care Management RN, you will serve as a vital liaison between patients, families, and healthcare providers. You will develop individualized care plans in collaboration with an interdisciplinary team to ensure continuity of care, while advocating for appropriate resource allocation that adheres to regulatory guidelines and reimbursement criteria.
Your day-to-day impact involves evaluating healthcare utilization patterns to identify opportunities for improving efficiency and cost-effectiveness. You will work to minimize readmissions, prevent complications, and optimize care planning processes by collaborating with insurance providers, social workers, and other stakeholders to ensure timely authorization of services. Additionally, you will participate in quality improvement initiatives and interdisciplinary care conferences to promote evidence-based practices and enhance patient safety.
