As an Adult Care Management RN (PRN) within the OUMC Care Management department, you will serve as a vital link in coordinating comprehensive care for selected patient populations. Your mission is to ensure the efficient utilization of healthcare resources while advocating for the appropriate allocation of services to meet patient needs. This role exists to bridge the gap between clinical care and resource management, optimizing outcomes through evidence-based practices and continuous quality improvement.
In this position, you will assess, plan, implement, and evaluate care options in collaboration with an interdisciplinary team, patients, and their families. Your day-to-day involves conducting comprehensive health assessments, developing individualized care plans, and providing education on treatment strategies and self-care. You will act as a liaison between patients, families, insurance providers, and community resources to facilitate timely authorizations and ensure regulatory compliance. A key focus of the role is identifying opportunities to improve efficiency, minimize readmissions, and enhance patient safety.
