Key Responsibilities
- check_circleCollaborate with primary care teams to identify patients for behavioral health referrals
- check_circleScreen and assess patients for common mental health and substance use disorders
- check_circleProvide short-term counseling and evidence-based treatments to patients
- check_circleCommunicate changes in behavioral health care plans to primary care teams
- check_circleParticipate in regional efforts to advance the Behavioral Health Program through workshops and feedback
- check_circleDevelop care plans and facilitate referrals to psychiatric consultants and external resources
- check_circleDocument patient encounters and counseling sessions accurately in the electronic medical record
- check_circleCoordinate care with psychiatric consultants including warm handoffs for telehealth visits
Requirements
- verifiedMaster's Degree in mental health counseling/healing arts from accredited school
- verifiedHighest clinical level license (LSCSW, LCPC, or LMFT)
- verifiedExperience screening for mental health/substance use disorders
- verifiedExperience with assessment and treatment planning
- verifiedFamiliarity with brief structured intervention techniques
- verifiedUS work authorization
Nice to Have
Proficiency in providing brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Cognitive Behavioral). Working knowledge of differential diagnoses of common mental health and/or substance use disorders. Proficient PC skills.
