The Behavioral Health Specialist is a key resource for Care Teams across a region of Oak Street Health primary clinics. This role exists to provide care management services and evidence-based short-term psychotherapy to patients with behavioral health needs. You will assess patient needs, make referrals to appropriate resources, and provide short-term counseling and evidence-based treatments as indicated.
In this position, you will collaborate closely with primary care teams to identify appropriate patients for the Behavioral Health Program. You will coordinate care with a psychiatric consultant, including facilitating warm handoffs for telehealth visits and collaborating on care plans. The role involves developing relationships with primary care teams and consistently communicating changes in behavioral health care plans. Depending on the maturity and panel sizes of the clinics, you may cover multiple locations. There is also a potential option for remote work days based upon the achievement and maintenance of program metrics.
