As a Referral Coordinator, you serve as the first point of contact for participants, providing a warm, authentic, and compassionate welcome to our services. Your primary mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time and in the right way, while actively encouraging them to utilize our virtual care platform for a self-led journey.
In this position, you will support participants in making informed choices about their care pathways by exploring in-house, sub-contracted, and adjacent service options. Whether through face-to-face interactions or telephone support, you will foster a personable, non-judgmental, and sensitive environment that respects the social determinants of health and behavior change principles.
Your day involves receiving and processing service referrals through multiple channels, including email, text, web forms, and telephone. You will conduct effective triage and assessment to capture accurate data, manage the referral inbox, and ensure friction-free onward sign-posting. A significant portion of your time will be dedicated to proactive caseload management for unassigned participants, engaging them via phone, text, email, or our virtual platform to drive successful onboarding.
You will also handle sensitive data capture, including minimum data sets, consent forms, and vulnerability assessments, while ensuring compliance with safeguarding policies. Additionally, you will deliver brief interventions and advice in line with government guidelines, maintaining high standards of data accuracy and performance through regular audits and feedback loops.
