About the Role

As a Referral Coordinator, you serve as the warm, authentic, and compassionate first point of contact for participants entering our service. Your mission is to guide individuals through the triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time, encouraging them to utilize our virtual care platform for a self-led journey while supporting those who require alternative pathways.

In this position, you will facilitate informed choices regarding care options, exploring in-house, sub-contracted, and adjacent services. Your day-to-day involves processing referrals across multiple channels, managing a caseload of unassigned participants, and capturing sensitive data with accuracy and empathy. You will work within a framework of standard operating procedures to ensure high-quality service delivery, safeguarding, and continuous improvement.

Key Responsibilities

  • check_circleDeliver brief interventions and advice in line with government guidelines
  • check_circleEngage with unassigned participants via phone, text, and virtual platforms to drive intervention onboarding
  • check_circleGuide participants through the onboarding process and support informed care pathway choices
  • check_circleImplement standard operating procedures and adhere to data protection and safeguarding policies
  • check_circleConduct triage and assessment of participant needs, preferences, and goals
  • check_circleReceive and process service referrals via multiple channels including email, text, and telephone
  • check_circleManage participant data capture, including minimum data sets, consent, and vulnerability assessments
  • check_circleMonitor referral dashboards and manage caseloads to ensure timely service delivery
