About the Role
As a Referral Coordinator, you serve as the first point of contact for participants, providing a warm, authentic, and compassionate welcome to our services. Your primary mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will play a pivotal role in helping participants access the right intervention at the right time, while encouraging them to utilize our virtual care platform for a self-led journey.
In this position, you will support participants in making informed choices about their care pathways by exploring in-house, sub-contracted, and adjacent service options. Whether through a professional service delivery or sign-posting, your focus is on creating friction-free pathways that support a positive participant experience. You will manage a proactive caseload, engaging with individuals via telephone, text, email, and our virtual platform to drive successful onboarding and ensure no one is left without support.

Key Responsibilities

- check_circleEngage with unassigned participants via phone, text, and virtual platforms to drive intervention onboarding
- check_circleGuide participants through the onboarding process and support informed care pathway choices
- check_circleManage participant records and capture accurate data including consent and accessibility considerations
- check_circleImplement standard operating procedures and adhere to data protection and safeguarding policies
- check_circleConduct triage and assessment of participant needs, preferences, and goals
- check_circleReceive and process service referrals via multiple channels including email, text, and telephone
- check_circleMonitor referral dashboards and manage caseloads to ensure timely service delivery
Requirements
- verifiedProven track record in customer facing role (face to face and telephone)
- verifiedExperience with Case Management System
- verifiedExperience supporting vulnerable individuals through change process
- verifiedExperience with Microsoft Office
- verifiedExperience in health, community or social care environment
- verifiedStrong understanding of social determinants of health

Nice to Have
Relevant health coaching qualification or an accredited health coaching skills programme. Evidence of continuous professional development to ensure the delivery of the most recent and up to date health coaching provision to the correct quality standard. Seeks self-improvement.
