As a Referral Coordinator, you serve as the first point of contact for participants, providing a warm, authentic, and compassionate welcome to our services. Your primary mission is to guide individuals through our triage and assessment process, ensuring their unique needs, preferences, and goals are carefully managed. You will support participants in accessing the right intervention at the right time and in the right way, while actively encouraging them to utilize our virtual care platform for a self-led journey.
In this role, you will help participants make informed choices about their care pathways by exploring in-house, sub-contracted, and adjacent service options. Whether through a professional service, compassionate communication, or effective triage, your focus is on creating simple, clear, and friction-free pathways that support a positive participant experience. You will manage a proactive caseload, engaging with unassigned participants via telephone, text, email, and our virtual care platform to drive successful onboarding.
