Grace Health is dedicated to providing comprehensive, integrated care that addresses the physical, mental, and behavioral needs of our community. We believe that managing patient care is a true team effort, requiring seamless collaboration between clinical and non-clinical staff. Our mission is to improve psychosocial functioning for children, adolescents, adults, and families through a model that prioritizes population management and proactive care.
This position serves as a vital link within our integrated health care model, offering behavioral health consultation to a diverse range of clients. You will work alongside primary care teams to detect "at-risk" patients early, developing plans to prevent further psychological or physical deterioration. The role is designed for professionals who thrive in a fast-paced primary care environment and are comfortable working with an interdisciplinary team.
In this role, you will not only provide assessment, diagnosis, and treatment planning but also actively participate in meetings that shape our comprehensive care strategies. You will teach patients, families, and staff about care, prevention, and treatment enhancement techniques, while also supervising students at the Bachelor's and Master's levels. Your work will directly contribute to continuous quality improvement activities, ensuring that every patient receives high-quality, coordinated care.
At Grace Health, we embrace a team-based approach where every role is essential to our success. We value excellent communication and coordination, particularly when working with vulnerable patient populations. Our team members are expected to be flexible, efficient, and cooperative, working effectively with providers, administration, and peers. We are committed to advocacy and actively involve our staff in improving practice performance and quality outcomes.
Grace Health provides equal employment opportunities to all employees and applicants for employment. We prohibit discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state, or local laws.
