This position serves as a vital link within our integrated health care model, offering behavioral health consultation to a diverse range of clients. You will work alongside primary care teams to detect "at-risk" patients early, developing plans to prevent further psychological or physical deterioration. The role is designed for professionals who thrive in a fast-paced primary care environment and are comfortable working with an interdisciplinary team.
In this role, you will not only provide assessment, diagnosis, and treatment planning but also actively participate in meetings that shape our comprehensive care strategies. You will teach patients, families, and staff about care, prevention, and treatment enhancement techniques, while also supervising students at the Bachelor's and Master's levels. Your work will directly contribute to continuous quality improvement activities, ensuring that every patient receives high-quality, coordinated care.
