About the Role
The Primary Care Physician (PCP) at ChenMed is more than a consultant for sickness; you will act as a health coach dedicated to helping patients set and achieve short and long-term health goals. You will lead a care team to simplify and prioritize care, ensuring behavior change is actionable for both patients and the team.
In this role, you will demonstrate accountability for outcomes for a panel of up to 450 patients. You will work closely with managers and medical directors to ensure compliance with guidelines while participating in risk and quality management programs. You will lead your center's culture, championing a positive environment and collaborating with care promoters, facilitators, and coordinators to manage acute and complex chronic health needs.
You will function both independently and in collaboration with other healthcare professionals, engaging with hospitalists and transitional care teams to ensure seamless care for patients in hospitals, skilled nursing facilities, or homebound settings. As a leader in our organization, you will also play an active role in center management and recruiting.
