This Clinical Pharmacist position is designed to provide direct operational and clinical patient services while collaborating within the department to enhance operations and clinical programs. You will be a vital part of a multidisciplinary team, working to improve care, achieve optimal outcomes, and provide continuity of care for patients ranging from neonates to geriatrics.
In this role, you will go beyond traditional dispensing. You will be delegated the authority to obtain patient medical histories, order and evaluate laboratory results, and make critical medication decisions, including modifying or discontinuing treatment when appropriate within your scope of practice. Your focus will be on providing evidence-based, cost-effective comprehensive medication management for patients with chronic conditions, ensuring therapy is appropriate and effective.
The day-to-day balance between operational and clinical services will adapt based on patient requirements, staffing needs, and overall pharmacy workload. You will also serve as a mentor, facilitating learning for pharmacy students, residents, technicians, and new employees. This includes providing experiential education, evaluating training programs, and sharing clinical and scholarly activities to advance the profession of pharmacy.
