The Ambulatory Clinic RN Care Coordinator serves as a clinical expert within the Cancer Center at Westwood Medical Pavilion, dedicated to delivering ethical, evidence-based, and compassionate nursing care. This role is pivotal in ensuring patient safety and the quality of care by identifying patient needs, coordinating healthcare services, and evaluating outcomes. The Care Coordinator acts as a vital liaison between patients, families, front desk staff, nursing teams, and physicians, ensuring seamless communication and understanding of the plan of care throughout the clinic session.
In this position, you will support assigned physicians by managing patient flow, triage, and direct care. A key aspect of the day-to-day work involves developing relationships with patients to identify physical, psychosocial, financial, and environmental health needs. You will coordinate continuity of care for patients following hospital admissions, discharges, and ER visits, with a specific focus on managing high-risk populations and those with multiple co-morbidities. The role also emphasizes patient advocacy, self-management support, and the delivery of health education to patients and ancillary personnel regarding safety, medications, and wellness.
