Join a caring, dedicated nursing team making a difference in our communities as an Ambulatory Care Nurse (RN). You will serve as an integral member of the patient care team within our Primary Care Medical Home model. This approach is not defined by a special building, but by a commitment to putting the patient at the center of healthcare decision-making.
In this role, you will manage a caseload of patients identified as at risk for health deterioration, working closely with assigned care teams to coordinate services throughout the continuum of care. Your day will involve facilitating the implementation of prescribed medical treatment plans through patient-centered education on disease processes and wellness opportunities. You will empower patients to develop self-management skills, ensuring their medical and psychosocial needs are met while identifying barriers to adherence.
