As an Ambulatory Care Nurse (RN), you will join a caring and dedicated team making a tangible difference in our communities. You will serve as an integral member of the patient care team, functioning as the in-office nurse within our Primary Care Medical Home model.
Working at Westside means working in a Primary Care Medical Home—a philosophy that puts the patient at the center of healthcare decision-making rather than focusing on a specific building. In this role, you will support assigned care teams by managing high and rising-risk patients and coordinating services throughout the continuum of care. Your day will involve facilitating the implementation of prescribed medical treatment plans through patient-centered education on disease processes and wellness opportunities. You will empower patients to develop self-management skills, ensuring both their medical and psychosocial needs are met while identifying barriers to adherence.
