We are looking for a caring and dedicated Ambulatory Care Nurse (RN) to join our team. This is a part-time, on-site position where you will serve as an integral member of the patient care team within a Primary Care Medical Home. This model is not defined by a special building, but by a way of providing healthcare that puts the patient at the center of decision-making.
In this role, you will support assigned care teams by managing high and rising-risk patients and coordinating services throughout the continuum of care. You will facilitate the implementation of prescribed medical treatment plans through patient-centered education on disease processes and wellness opportunities. Your focus will be on developing patient self-management skills, empowering individuals to reach their health goals, and ensuring both medical and psychosocial needs are met.
