About the Role

This position serves as a clinical leader within our Brooklyn home visit program, dedicated to promoting compliance with quality and cost measures while upholding the highest standards of care. You will manage the service delivery of inter-professional and para-professional teams, acting as a key resource to provide clinical and operational guidance that enhances team outcomes.

In this per diem role, you will provide advanced nursing clinical care for patients in accordance with current State and Federal rules, regulations, and national standards of care. Working under the supervision of the Clinical Director for the Nurse Practitioner Program, you will evaluate patient responses to therapy, revise inter-professional plans of care as necessary, and collaborate with physicians to develop and train staff on new clinical protocols.

Your day-to-day involves managing comprehensive advanced nursing care, including physical examinations, comprehensive histories, and emergent interventions. You will establish treatment plans based on clinical findings, determine when further evaluation by specialists is warranted, and provide intensive care management across client settings. This role requires a deep commitment to the Patient-Centered Medical Home standards and the meaningful use of medical record data.
