The Adult Care Management Social Worker (PRN) serves as a vital link within the Emergency Department and OU Health Care Management team. This role is dedicated to managing care for high-risk members navigating complex chronic behavioral and health conditions. You will collaborate closely with members, caregivers, physicians, and the interdisciplinary healthcare team to ensure timely access to necessary care, continuity across all settings, and shared decision-making.
In this position, you will provide essential support and guidance to patients and families as they navigate wellness within the OU Health system. Under the supervision of a Social Worker II, you will act as a liaison between patients and clinical staff, advocating for patient concerns and connecting individuals to supportive services and community resources. The role requires a commitment to fostering a culture of continuous learning, quality improvement, and patient-centered care.
Our mission is to ensure that every patient receives compassionate, coordinated care that addresses their medical, social, and emotional needs. As part of the Care Management team, you will lead interdisciplinary rounds and team meetings, driving performance improvement initiatives through data collection and research. You will also play a key educational role by precepting newly hired staff and serving as a field instructor for social work interns from bachelor and master's programs.
This is a PRN (as needed) position requiring a minimum commitment of 2 shifts per month. We offer market-leading rates for these roles.
