As an Adult Care Management Social Worker within the Inpatient SICU/NICU department at OU Health, you will provide essential support and guidance to patients and their families navigating complex social and health-related challenges. This role is designed to ensure comprehensive care coordination, helping individuals achieve overall wellness within the OU healthcare system.
You will work collaboratively with clients, families, healthcare professionals, and community resources to address unique needs and improve well-being. Your day-to-day involves assessing patient conditions through medical diagnoses and personal interviews, conducting psychosocial assessments, and developing care plans that include resource referrals, crisis intervention, and patient advocacy. You will serve as a vital liaison between patients and clinical staff, ensuring that ethical dilemmas are escalated appropriately and that patients are treated with dignity and respect.
This position may be filled as Level 1, 2, or 3, depending on your specific education, experience, and license requirements. Level 1 roles may also involve precepting newly hired social workers or students.
