As an Adult Care Management Social Worker within the OU Health Inpatient (Med-Surg) department, you will serve as a vital bridge between patients, their families, and the broader healthcare system. This role is designed to provide essential support and guidance as individuals navigate complex social and health-related challenges. You will work collaboratively with clients, families, and healthcare professionals to address unique needs, ensuring a comprehensive approach to overall wellness.
In this position, you will conduct psychosocial assessments, coordinate care plans, and advocate for patient concerns. Your day-to-day involves acting as a liaison between clinical staff and external agencies, providing crisis intervention, and offering education on diseases, treatments, and emotional stressors. You will also play a key role in high-risk screening and documenting sensitive issues such as abuse or neglect, all while maintaining an open, respectful, and dignified approach to every interaction.
Depending on your specific education, experience, and licensure, this position may be filled as a Level 1, 2, or 3. This flexibility allows us to match the role's scope with your professional background, whether you are a newly licensed social worker or an experienced clinician.
