As a Licensed Master's Social Worker (LMSW) at GLBHC, you will serve as a vital link in our integrated care model, addressing the behavioral health, substance use, and social determinants of health needs of our diverse patient population. This role exists to ensure that patients receive timely, collaborative, and comprehensive support whether they are in integrated, traditional, or specialized care settings.
Your day will involve accepting referrals from the care team and responding with effective, collaborative communication. You will conduct comprehensive assessments, provide diagnoses, and deliver interventions ranging from individual counseling to group therapy for children, adolescents, and families. A significant part of your work involves collaborating with the care team to develop care management processes that help patients manage chronic conditions and detecting "at-risk" patients to prevent further deterioration.
You will be an active participant in huddles, staff meetings, and case conferences, ensuring that your practice aligns with GLBHC's behavioral health care models. By maintaining accurate documentation and monitoring behavioral health needs at your assigned site, you will help identify gaps and opportunities for improvement. This position requires a positive, professional attitude and a genuine passion for serving our patients.
Note: If hired as an LMSW, you are eligible for a $3,000 signing bonus.

Previous substance abuse experience based upon grant or position specifications.
