As an Adult Care Management Social Worker within the Inpatient Med-Surg Department at OU Health, you will serve as a vital bridge between patients, their families, and the broader healthcare system. This role is designed for individuals passionate about navigating complex social and health-related challenges to improve overall patient well-being. You will provide essential support, guidance, and comprehensive care coordination, working collaboratively with healthcare professionals and community resources to address unique patient needs.
In this position, you will conduct psychosocial assessments, develop care plans, and offer crisis intervention and advocacy. Whether you are a newly licensed social worker or an experienced clinician, you will play a key role in educating patients on diseases and treatments, connecting families with resources, and ensuring that ethical concerns are addressed with dignity and respect. The position may be filled at Level 1, 2, or 3, depending on your specific education, experience, and licensure.
