As a Pediatric Care Management Social Worker within the NICU Department at OU Health, you will provide essential support and guidance to patients and their families as they navigate the complexities of the healthcare system. This role is dedicated to delivering comprehensive care coordination and support services to individuals facing unique social and health-related challenges. You will work collaboratively with clients, families, healthcare professionals, and community resources to address specific needs and improve overall well-being.
In this position, you will assess patients' unique conditions by synthesizing data from medical diagnoses, chart reviews, and personal interviews. You will perform psychosocial assessments, relate findings to clinical staff, and supervise or provide plan of care interventions. These interventions include resource and referral assistance, crisis intervention, prevention, education, patient advocacy, bereavement support, and therapy (for LMSW under supervision). You will act as a vital liaison between patients and families and the clinical team, advocating for patient concerns and escalating ethical dilemmas through appropriate channels.
The role requires maintaining an open, approachable manner that treats others fairly and respectfully, preserving self-confidence and dignity. You will also complete reports based on mandated reporting requirements and may serve as a preceptor for newly hired social workers or students (BSW and MSW) if you hold an LMSW or LCSW with one year of experience.

This position may be filled as Level 1, 2, or 3 depending on specific education, experience, and license requirements. The schedule is Monday through Friday, 8:00 AM to 4:30 PM.
