The Admissions Coordinator plays a critical role in facilitating timely and accurate hospice admissions by coordinating referrals, patient records, physician orders, and patient/family appointments. This position serves as a key liaison among Community Liaisons, referral sources, patients, families, and our clinical team.
In this capacity, you will be a compassionate and knowledgeable point of contact during the admission process, explaining hospice services, benefits, and eligibility to patients, family members, and referral partners. You will ensure that the interdisciplinary team is promptly notified of new referrals, admissions, and non-admissions, while coordinating the necessary supplies and DME provision to meet patient needs.
