About the Role
Crossbridge Hospice is seeking an Admissions Coordinator to facilitate compassionate, timely hospice admissions. In this role, you will serve as the first point of contact for patients, families, and referral sources, coordinating assessments, verifying eligibility, and ensuring accurate documentation. You will collaborate closely with our interdisciplinary hospice team to support seamless transitions of care, primarily for patients in home and community settings.
As the primary liaison for new referrals, you will provide compassionate guidance to families navigating this significant life transition. Your day will involve scheduling admission assessments, communicating with physicians and community partners to streamline the referral process, and educating families on hospice services and available support resources. By maintaining meticulous records in our EHR systems and monitoring pending referrals, you will ensure that no patient experiences delays in receiving the care they need.

Requirements
