Our mission is to keep patients at home and out of the hospital while maximizing their comfort, dignity, and quality of life. Across the Care Synergy network, we serve thousands of individuals and families annually through hospice, home health, palliative care, and wellness programs, generating significant community impact with over 7,000 hospice patients served each year.
As an Admissions Coordinator I, you will be the vital link between our community and our compassionate care services. You will provide excellent service to internal and external customers regarding hospice, palliative, and home health programs. Your day will involve coordinating the intake of referrals and admissions, ensuring patients meet criteria and receive the appropriate care. You will act as an ambassador for our organization, answering calls, explaining care options to patients and families, and collaborating with the Business Development and Clinical teams to ensure timely patient care. This role is essential in guiding families through the admission process, verifying insurance coverage, and initiating necessary paperwork with empathy and efficiency.
