About the Role

As a Wellness Nurse, you will join a team committed to monitoring the health and wellness of our residents. This role exists to ensure the overall well-being of our community members by placing the resident first in every interaction. You will develop meaningful relationships with residents and their families, supporting their physical, spiritual, emotional, and clinical needs through a consistent caregiver approach.

In this position, you will serve as a strong advocate for our seniors, conducting observations and evaluations to identify changes in clinical conditions. Your day-to-day impact involves updating service plans, reconciling medication deliveries, and collaborating with Assisted Living, Memory Care, and Wellness Managers to analyze care outcomes. You will also coordinate ancillary support services, including Home Health, Hospice, and provider visits, ensuring that all medical needs are met with the highest achievable level of care.
