About the Role

We are seeking a dedicated Wellness Nurse to join our team on a part-time basis for the afternoon shift (3:00 PM to 11:30 PM), working 16 hours a week on an every-other-week rotation. In this role, you will be the primary advocate for the health and wellness of our residents, ensuring their overall well-being is prioritized at every turn.

Your day will involve developing meaningful relationships with residents and their families using a consistent caregiver approach that supports physical, spiritual, emotional, and clinical needs. You will monitor medication and treatment profiles to ensure accuracy and safety, while also observing and evaluating residents for changes in clinical condition. Beyond clinical duties, you will collaborate with Assisted Living, Memory Care, and Wellness Managers to analyze care outcomes and coordinate ancillary support services such as home health, hospice, and rehab. This role is vital for maintaining the highest achievable level of well-being for our community members.
