About the Role

We are seeking a dedicated Wellness Nurse to join our team on a part-time basis for the afternoon shift (3:00 PM to 11:30 PM), working 16 hours a week on an every-other-week rotation. In this role, you will be the primary advocate for the health and wellness of our residents, ensuring their overall well-being is prioritized at every turn.

Your day will involve developing meaningful relationships with residents and their families, supporting their physical, spiritual, emotional, and clinical needs through a consistent caregiver approach. You will monitor medication and treatment profiles, conduct observations to identify changes in clinical conditions, and update service plans as needed. Additionally, you will collaborate with Assisted Living, Memory Care, and Wellness Managers to analyze care outcomes and coordinate ancillary support services such as hospice, home health, and outpatient rehab.
