The Wellness Care Manager serves as a vital link between patients, caregivers, and the broader healthcare ecosystem. In this position, you will coordinate home and community-based care, ensuring that every individual receives seamless, patient-centered support. Your day will involve conducting comprehensive biopsychosocial assessments and developing individualized wellness and care plans tailored to unique clinical and social needs.
You will act as a dedicated advocate, addressing barriers to care such as housing, transportation, or financial constraints while connecting individuals with essential resources. By collaborating closely with nurses, physicians, social workers, therapists, and community partners, you will facilitate smooth transitions between hospital, home, and rehab settings to reduce avoidable readmissions.
