About the Role
We are seeking a compassionate and detail-oriented Triage & Care Coordination Specialist to serve as the central communication hub for our Skilled Nursing Facility (SNF) and triage service lines. In this role, you will bridge the gap between patients, providers, facilities, and interdisciplinary teams to ensure timely care coordination, improved patient outcomes, and seamless transitions of care.
This position is ideal for a professional who thrives on patient advocacy and problem-solving within value-based care environments. You will play a vital part in quality improvement initiatives, helping to refine workflows that reduce gaps in care and enhance the overall patient experience.

Key Responsibilities

- check_circleSupport follow-up on orders, care plans, and provider recommendations
- check_circleIdentify barriers to care and promote patient self-management
- check_circleTrack quality and program metrics using internal tools and dashboards
- check_circleHelp refine workflows to improve care transitions and reduce gaps in care
- check_circleParticipate in quality improvement and patient satisfaction initiatives
- check_circleParticipate in interdisciplinary meetings and case discussions
Requirements
- verified2+ years of healthcare, care coordination, or patient support experience
- verifiedBachelor's degree preferred
- verifiedBilingual Spanish preferred
Nice to Have
Experience in SNF, post-acute, managed care, or value-based care environments. Bachelor's degree. Bilingual Spanish skills.
