As a Travel Registered Nurse in Home Health, you will provide skilled nursing care to patients in their homes throughout Greensburg and the surrounding rural areas. This role is ideal for an RN who values one-on-one time with patients, enjoys building long-term relationships, and wants to see the direct impact of their care on patients' comfort, safety, and independence.
You will practice with a high level of autonomy while remaining supported by a collaborative team. Your day will involve performing comprehensive patient assessments, developing individualized plans of care, administering medications and treatments, and monitoring chronic conditions. You will also provide education to patients and caregivers on disease management, medications, and home safety. The patient population is diverse, primarily consisting of adults and geriatric patients recovering from surgery, managing heart failure, COPD, diabetes, wound care needs, or rehabilitation after hospitalization.
Visits are scheduled to allow for thorough assessments and meaningful education rather than rushed encounters. While the primary hours are daytime shifts to align with patient and family schedules, you may have some flexibility to arrange visit routes across the area. The work is primarily field-based, traveling from home to home, with time spent on documentation and coordination using an electronic medical record system. You will join a supportive team that values collaboration, open communication, and shared problem-solving, with orientation and ongoing education provided to ensure comfort with home health regulations and specialized field skills.
