TravelNurseSource is partnering with AMN Healthcare to find a qualified Registered Nurse for a Home Health assignment in Jeffersonville, Indiana. This 13-week contract, starting ASAP, offers a unique opportunity to deliver skilled nursing care within a welcoming riverfront community just across the Ohio River from Louisville, Kentucky.

About the Role

As a Home Health RN, you will provide skilled nursing care to a diverse adult and geriatric population in their own homes. This role offers a high level of autonomy and meaningful one-on-one time, allowing you to build strong relationships while supporting patients in maintaining independence. You will manage complex medical conditions such as heart failure, COPD, diabetes, post-surgical recovery, and wound care needs.

Your day will involve managing a caseload of home visits within an assigned territory, conducting start-of-care assessments, routine follow-ups, and discharge visits. You will collaborate closely with an interdisciplinary team—including physical, occupational, and speech therapists, social workers, and physicians—to coordinate comprehensive, patient-centered care. Documentation will be handled via electronic medical record systems, supported by office-based coordination staff. The schedule generally features daytime hours with flexibility to accommodate patient needs and rotating on-call responsibilities.
