As a travel professional, you will spend 14 weeks providing critical support to patients navigating complex medical journeys. Your day will involve conducting thorough psychosocial assessments, delivering supportive therapy, and executing comprehensive discharge planning, including referrals management. This role exists to ensure seamless transitions of care and emotional support for patients and their families during their hospital stay.
