About the Role
In collaboration with attending physicians, the Geriatric Nurse Practitioner will deliver comprehensive care to patients across one or more clinical venues at Hebrew Rehabilitation Center. The position focuses on the management of both chronic and episodic illnesses, ensuring high-quality, patient-centered outcomes.
This role is designed for a collaborative practitioner who thrives in a multi-disciplinary environment. You will establish a collaborative primary care practice with Primary Care Physicians (PCPs), working closely with physicians and team members to manage complex medical needs. The position requires a professional who can effectively communicate, listen actively, and foster an ethical, supportive work environment while maintaining strict attention to detail in documentation and data accuracy.
Hiring Process
Candidates must hold a Master's Degree from an accredited Nurse Practitioner program and possess a valid license as a nurse practitioner by the Commonwealth of Massachusetts Board of Registration in Nursing. National certification as a Geriatric, Adult, or Family Nurse Practitioner is required. While four years of Nurse Practitioner experience and two years in a geriatric facility are preferred, the primary focus is on clinical competence and the ability to work effectively within a team to achieve desired results.
