As a Targeted Case Manager, you will join a mission-driven team passionate about making a difference in the lives of individuals with developmental disabilities. This role exists to bridge the gap between individuals and the resources they need, utilizing advocacy, resources, and kindness to support their journey.
In this position, you will manage a moderate-sized caseload, serving as a dedicated point of contact for those you serve. Your day will involve facilitating individual support planning, coordinating cases, and linking individuals to essential resources. You will work collaboratively to ensure that every individual receives the support necessary to thrive, maintaining regular contact and ensuring all documentation is accurate and complete.
