As a Supports Coordinator, you will serve as a vital liaison between individuals, families, service providers, and government agencies. Your day-to-day work involves developing and managing Individual Support Plans (ISPs) using a person-centered approach, coordinating services to ensure quality and effectiveness, and advocating for individuals to achieve their personal goals while promoting the values of "Everyday Lives." You will conduct face-to-face and telehealth visits throughout Bucks County, maintaining accurate documentation in an electronic health record system.
