The Street Outreach Coordinator serves as a vital bridge between the community and healthcare services. In this field-based role, you will engage individuals experiencing homelessness and other vulnerable populations through consistent street and site-based outreach. Your primary goal is to build trust, reduce barriers to care, and connect participants to essential healthcare, social services, and supportive resources.
You will operate across multiple locations in the City of Chicago, including shelters, encampments, drop-in centers, and community sites. The role blends direct community engagement with clinical support, assisting with participant registration in the agency EHR system, verifying insurance coverage, and documenting outreach encounters. This position is designed for someone who thrives in dynamic environments and is committed to trauma-informed, non-judgmental interactions.
