We are seeking an experienced and compassionate Hospice Clinical Liaison to join our team in Dallas, Texas. In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization. You will play a vital part in building strong professional relationships, coordinating admissions, and supporting community awareness of hospice services.
As a liaison, you will serve as the bridge between referral sources and our hospice care team, ensuring seamless handoffs and clear communication. Your day will involve making daily visits to hospitals and facilities to maintain visibility with discharge planners, case managers, and physicians. You will educate patients and families on what hospice care includes, providing clarity, reassurance, and compassion during critical moments.
