About the Role

As a Home Health Speech Language Pathologist, you will partner with a multidisciplinary team of nurses, physical therapists, occupational therapists, home health aides, and medical social workers. Together, you will help patients rehabilitate, recover, and regain their independence so they can live healthier and happier lives. Your work involves evaluating, directing, and providing speech/language pathology services to patients in home or facility settings, utilizing professional judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing.

You will be instrumental in the development and periodic review of the Plan of Treatment and Plan of Care, administering diagnostic tests, and providing instruction to patients, families, and healthcare staff on alternative communication systems. Your role requires maintaining the highest standards of professional conduct regarding patient confidentiality and participating in care coordination activities and discharge planning.
