About the Role
We are seeking a dedicated Speech Language Pathologist to join our home care team in the Bronx. In this role, you will deliver high-quality, patient-centered therapy to adults in their homes, helping them regain or maximize functional communication and safety in daily life. You will work closely with an interdisciplinary healthcare team to assess, plan, implement, and evaluate individualized treatment plans that reflect each patient's medical condition, goals, and cultural background.
As a home care clinician, you will manage your own caseload and daily schedule, adapting your approach to meet patients where they are—physically and emotionally. Your work will involve conducting comprehensive assessments and providing evidence-based interventions for conditions such as stroke, neurological disorders, respiratory compromise, voice disorders, and cognitive-linguistic impairments. You will also educate patients, families, and caregivers on home exercise programs, compensatory strategies, and safe swallowing techniques, empowering them to actively participate in care and recovery.
