We are seeking a dedicated Speech Language Pathologist to provide essential home-based care to adult patients across the Bronx. This role is vital for assessing, diagnosing, and treating speech, language, cognitive-communication, and swallowing disorders. You will work directly with patients and their families to develop and implement individualized, evidence-based treatment plans that support progress in a home setting.
In this position, you will be the bridge between complex medical needs and daily life. Your day will involve conducting thorough evaluations, educating caregivers on safe swallowing techniques and communication strategies, and coordinating closely with an interdisciplinary team of physicians, nurses, and therapists. You will manage your own caseload efficiently, navigating varied home environments to deliver personalized care while adhering to strict safety and ethical standards.
