We are seeking a dedicated Speech-Language Pathologist to deliver essential home-based care to patients across the Bronx. This role is vital for assessing, diagnosing, and treating speech, language, cognitive-communication, and swallowing disorders in patients of all ages. You will serve as a key partner in the patient's journey, developing and implementing individualized, evidence-based treatment plans that align with specific patient goals and safety needs.
Your day will involve traveling throughout the Bronx to conduct comprehensive evaluations and provide direct therapy for disorders including aphasia, dysarthria, apraxia, and dysphagia. Beyond clinical interventions, you will educate patients, families, and caregivers on communication strategies, compensatory techniques, and safe swallowing practices. You will also design home exercise programs to support functional communication and swallowing in daily life. Working within an interdisciplinary home care team, you will participate in case conferences, coordinate with physicians, and advocate for necessary equipment or services to ensure patient safety and progress.
Candidates who meet the licensing and certification requirements will be invited to discuss their clinical experience and approach to home care. The process includes a review of credentials, followed by an interview with the hiring team to assess cultural fit and clinical expertise. Successful candidates will then complete background and health clearance requirements prior to onboarding.
