This position is a combined posting for a Social Worker MSW (CSWA) and a Licensed Clinical Social Worker (LCSW) at Providence St. Vincent's Medical Center in Portland, Oregon. The role operates on an on-call, day schedule as part of the Care Management department.
Social workers in this role provide comprehensive biopsychosocial and spiritual assessment, diagnosis, treatment, interventions, and advocacy for patients and their support systems. Whether working with inpatient or outpatient populations, you will incorporate concepts of life span development, neurobiology, and behavioral change. You will collaborate with patients and their support systems to develop and implement evidence-informed, person-centered care plans across the care continuum.
Working closely with the Inter/Multi-Disciplinary Team (IDT) and various departments throughout the organization, you will ensure duties are performed in a manner that respects patient self-determination, reduces health disparities and barriers to care, and adheres to the NASW Code of Ethics and Standards of Practice. The CSWA role operates under direct and continuous supervision, while the LCSW role independently provides these comprehensive clinical services.
