Do you have a massive heart for empowering people with intellectual and developmental disabilities to live their absolute best lives? Are you the kind of person who can chat with anyone, solve puzzles on the fly, and thrives when working with a team?
If you just nodded your head enthusiastically, we want you! Join Mosaic as a Mosaic at Home Coordinator and become a professional difference-maker.
At Mosaic, we are relentlessly pursuing opportunities that empower people to live meaningful lives in caring communities. As a Coordinator, you aren't just managing cases—you're the ultimate bridge builder, ensuring the people we serve have a powerful voice and personalized support every single day. You will be the go-to person for developing rock-solid relationships with the individuals we support, their families, host home providers, and direct care workers. Your role involves helping individuals express their goals, dreams, and needs, ensuring their voices are not just heard, but loudly respected.
You will monitor relationships between individuals and their home providers to ensure perfect, positive, and supportive matches. Collaborating with the service planning team, you will weave together a person's unique wishes with necessary policy and regulatory requirements to create Individual Support Plans. You'll also keep it real in the field by overseeing service delivery and contract compliance through regular home and service environment visits. Additionally, you will provide on-call support to our wonderful providers and direct care workers when they need a helping hand or a quick problem solved.
This is a salaried, daytime position based at our office in Hastings, Central Nebraska. The role includes on-call responsibilities for the providers and individuals on your caseload, which can involve additional hours outside of normal business hours. You will hit the road for monthly home visits, which can be a minimum of 6 hours from Hastings, using an agency vehicle or your own (with mileage reimbursement).
At Mosaic, we believe in creating a workplace where everyone has the chance to contribute and succeed. This commitment is not just a policy; it's the way we work. It's good for the workforce, it's good for Mosaic, and it's the right thing to do. We don't just put our values on a poster; we live our values of belonging, connection, faithfulness, and grit every day.
You get to work for a mission-driven organization that is actively creating inclusive communities. Join a compassionate, collaborative team that genuinely values your ideas and contributions. We love to see our people shine and offer great opportunities for training and career advancement. We offer comprehensive benefits after 60 days, including health, dental, prescription, life insurance, and short/long-term disability. We also provide tuition assistance, a 403(b) retirement plan, and PTO that starts accruing on Day 1.
